Grimace's Birthday is a side-scrolling platformer that spans four levels across two stages. In the first stage, players control Grimace on a skateboard, executing rail grinds and tricks. The subsequent stage allows Grimace to move freely, aided by a bubble power-up for graceful floating. Points are earned within a time limit, with actions like defeating adversaries and executing tricks contributing. Collecting 40 Grimace Shakes adds bonus points. After completing the stages, players participate in a bonus minigame, earning extra points by extinguishing candles on Grimace's birthday cake.
Beyond the main story mode, Grimace's Birthday offers two additional game modes including the "Score Attack" and the "Freeskate". The "Score Attack" mode challenges players to amass as many points as possible within a set time frame, while the "Freeskate" mode empowers players to navigate freely without the constraints of a time limit.
